                  Parent's Forms

Once you have accepted our childcare, you will be asked to complete a series of forms. These forms will orientate from both the state and our internal business processes.

 Included with these forms, you must also provide the child's shot record. Please see below for more information.

Note to Parents: Children will not be accepted into my childcare without an up-to-date shot record. You have one-week after enrollment to submit these records. Failure to submit shot records will terminate our contract and your child will not be allowed to return to the childcare.

In addition to the above forms required, if you are enrolling your child for night-time care, you will be subject to a background check. This is to ensure that our home will not be compromised or endangered with criminal history or behavior.
